PASSPORT stands for Pre-Admission Screening Providing Options and Resources Today. This program offers home health care services to eligible individuals to help them live at home instead of other care facilities. It is funded by the State and federal funds under the Medicaid Waiver III program.
PASSPORT care may cover the following services to an eligible individual:
- Nursing care
- Physical, occupational and speech therapy
- Personal care
- Respite care
- Social work/counseling
- Homemaker services
- Home-delivered meals
- Nutritional consultation
- Chore services
- Adaptive and assisting devices (such as crutches)
- Home medical equipment
- Medications
- Home health case management.
The case manager will always conduct assessments to make sure your needs are met. They will monitor your health and adjust the plan of care as needed.
